Who is eligible to participate in the ActiveHealth program?
The ActiveHealth program is open to State of Indiana employees, spouses, and adult dependents enrolled in a State Personnel Department sponsored medical plan. Employees and spouses who participate in the ActiveHealth program can earn wellness rewards.
Why do I have to be a member of the health plan to participate? The ActiveHealth program is a coordinated benefit within the health plan; therefore, it is only available to employees, spouses, and adult dependents who are enrolled in a health plan sponsored by the State Personnel Department.
Who can earn wellness rewards? Employees and spouses that are eligible for the ActiveHealth program.
I am a new employee, can I participate? Yes, if you enroll in a health plan sponsored by the State Personnel Department. New employees are encouraged to begin participating in all wellness programs available to them. New employees typically have access to create an ActiveHealth account 1-3 weeks after health plan coverage begins.

New employees are encouraged to begin participating in all wellness programs available to them. New employees typically have access to create an ActiveHealth account 1-3 weeks after health plan coverage begins.
How do I create an ActiveHealth account?
Go to www.myactivehealth.com/StateofIndiana and click on “Sign up for MyActiveHealth” at the bottom of the page. Enter your name as it appears on your insurance card, date of birth, and zip code.
If you get a message that says “Error. You are already registered with MyActiveHealth” you can use the Forgot Username or Password links to reset your information.
If you get a message that says “Error. We are unable to locate your information at this time.” either the information entered could not be matched, or ActiveHealth has not received your information yet. Verify that you have entered your information as it appears on your insurance card, and that it has been 1-3 weeks since your health plan coverage began. You can call the SPD Benefits Hotline at 877-248-0007 to verify your health plan coverage start date.
Where do I go to login to the ActiveHealth Portal? Go to www.myactivehealth.com/StateofIndiana.
Is there an ActiveHealth app? Yes. You can get the app in the App Store or Google Play on your mobile device. If you have issues downloading the app, contact ActiveHealth Tech Support at (888) 588-8741, or email mobilesupport@aetna.com.

Do activities completed in 2023 count toward 2024 rewards?
Only activities completed in 2024 are eligible for rewards.
Can I complete activities on state time? While you cannot complete activities on state time, you are encouraged to work with your manager to use the WHOLE policy in order to flex time to complete rewardable wellness activities.

Will the gift cards be taxed? Yes, gift cards are taxable income based on IRS regulations. The value of the gift card will be included in your gross wages with a description of “Wellness Reward” on a paycheck following the redemption of the gift card.
What is the difference between earning and redeeming a gift card? An earned gift card means that you have completed an eligible activity and you are eligible to receive a gift card. A redeemed gift card means you have logged in to the ActiveHealth portal and used Rewards Center to select a retailer to receive your gift card.
How do I redeem my gift cards? Gift cards can be redeemed in the ActiveHealth portal in the Rewards Center. Click on “Redeem My Rewards” located under the progress tracker. What is the wellness reward for completing an annual physical?
Employees and spouses eligible for the ActiveHealth program can earn $200 for completing an annual physical with lab work and submitting the Annual Physical Results Form to ActiveHealth.
How long does it take for the activity to show as completed in the ActiveHealth Rewards Center? This activity will show as completed within four weeks of your Annual Physical Results Form being submitted to ActiveHealth and accepted for processing. To be accepted for processing, forms must be legible and fully completed including all required values and required signatures.
What if my health care provider doesn’t recommend lab tests for some of the values on the Annual Physical Results Form? If your provider determines some lab test values are not necessary for you, the provider can note that on the Annual Physical Results Form and the form will be accepted if noted properly Height, Weight, and Blood Pressure are always required to receive the reward.
What is an annual physical? It is an annual visit with your health care provider to assess your current health, identify any necessary preventive care, and review health changes over time.
What should I expect at an annual physical? At your annual physical, your provider will review your family and medical history, check your vital signs, order lab testing, and discuss lifestyle habits. Your provider will answer your health-related questions and offer guidance to improve your overall health and wellbeing.
How do I prepare for an annual physical? Create a list of all medications you currently take, including over-the-counter drugs and supplements. Include a list of any symptoms or pain you are experiencing and any questions you would like answered.
How do I find an in-network provider for an annual physical? The Anthem Health Guide team can help you find an in-network provider by calling 877-814-9709. The Anthem app, called Sydney, also has a provider search tool and an Anthem Health Guide chat option.

What is the deadline for earning the annual physical reward? The activity must show as completed in the ActiveHealth Rewards Center by Dec. 31, 2024 to earn the reward. All gift cards must be redeemed by Dec. 31, 2024. Keep in mind processing time is up to four weeks from the date the form is accepted to when the activity is shown as completed in the ActiveHealth portal. Keep in mind providers are required to bill for all services provided during an appointment. If non-preventive care is provided at the same time as the annual physical, there could be out of pocket costs.

How long does it take for the Cancer Screening activity to show as completed in the ActiveHealth Rewards Center? This activity is completed based on your provider submitting a claim for the service to Anthem. The activity will show as completed within two to three weeks of your claim being processed by Anthem.
What cancer screenings are rewardable? Breast cancer, cervical cancer, colon cancer, prostate cancer, and lung cancer screenings are rewardable activities in 2024.

We understand that not all screenings are right for everyone, and not all are recommended to be done annually. This is why there are lots of activity options to earn the $500 in gift card rewards.
How do I report that I’ve received a cancer screening? You do not need to report anything. The activity will show as completed within two to three weeks of your claim being processed by Anthem.
If you did not use your State Personnel Department sponsored health plan when receiving your screening, you will need to follow the Missing Preventive Services Form instructions. You can find the instructions and a link to the online form in the ActiveHealth Rewards Center in the activity tile for the screening that is missing. This form must be submitted by November 30 to allow for processing time.
Why isn’t my reward for my cancer screening showing in the ActiveHealth portal? You may be checking too soon. Cancer screening rewards are automatically completed within two to three weeks of your claim being processed by Anthem. Once you can see your claim has been processed at Anthem.com, expect to see your reward in ActiveHealth within two weeks.
Although uncommon, sometimes providers do not submit claims right away, or there is a problem with the way a claim was filed. If you’ve waited more than four weeks from the date of your cancer screening and Anthem still hasn’t processed the claim, you can use the Missing Preventive Care Service process to submit documentation. You can find the instructions and a link to the online form in the ActiveHealth Rewards Center in the activity tile for the screening that is missing. Documentation will not be processed unless it has been four weeks since the date of your service. This form must be submitted by November 30 to allow for processing time.
If you did not use your State Personnel Department sponsored insurance when receiving the service, you can use the same Missing Preventive Care Service process.
How do I know if Anthem has processed my claim? Log in to your Anthem.com account or Sydney app to view your processed claims. You can also call Anthem at 877-814- 9709.
When should I use the Missing Preventive Service Form? The Missing Preventive Services Form should be used in the following situations:
1. The claim was processed by a non-SPD insurance plan (for example, VA Health Benefits, or a free community health clinic); or
2. It has been four weeks since the date you received the service and Anthem has not yet processed the claim.

How long does it take for the Dental Cleaning activity to show as completed in the ActiveHealth Rewards Center? This activity is completed based on your provider submitting a claim for the service to Anthem. The activity will show as earned within two to three weeks of your claim being processed by Anthem.
What is a dental cleaning? A dental cleaning includes an exam by a dentist and scaling and polishing your teeth to remove any plaque and tartar build up. It is an important part of maintaining oral health. Dental cleanings should be completed every six months.
Who should get a dental cleaning? It is recommended that all adults have a dental cleaning with exam every six months. View the Anthem Dental Preventive Care flyer.
